  • Barriers to Action: What got in the way of doing the needed or expected effective behavior right away?

By answering these questions, people can pinpoint specific areas of deficiency—whether in knowledge, motivation, or situational awareness—that need enhancement or correction. This approach encourages a thorough investigation into the moments leading up to and following a problematic behavior.
